Why 1 in 4 Physicians is Ready to Leave Health-System Job

A recent report reveals that physicians working in health-system-led practices are significantly more dissatisfied than those in physician-led organizations. According to Bain’s Frontline of Healthcare Survey, health-system-led practices score 25 to 40 points lower on employee satisfaction than physician-led ones. This shift toward health system ownership of medical practices has led to declining satisfaction among physicians, who now face issues such as inadequate staffing, heavy workloads, and lack of autonomy. Nearly a quarter of physicians in these systems are considering switching employers, with 37% seeking to move to physician-led practices.

The report highlights that physician-led practices succeed by focusing on critical areas like fair compensation, leadership alignment, and meaningful involvement in decision-making. Physicians in these settings also report better access to resources and a stronger sense of belonging, contributing to higher retention and overall satisfaction. Health systems, however, can improve by implementing changes such as engaging physicians in strategic decisions, enhancing digital tools, and adopting a closed-loop feedback system. Embracing these best practices could foster a culture of trust and improve the physician experience in health-system-led organizations.
