NYRx Cost Optimization Program Update

The New York State Medicaid Pharmacy Program (NYRx) has expanded its Cost Optimization Program, effective July 16, 2026, by adding additional high-cost drug formulations and dosages that are significantly more expensive than therapeutically equivalent alternatives. The initiative targets medications where lower-cost strengths, dosage forms (such as tablets instead of capsules), or comparable drugs in the same class provide the same clinical benefit. Prescribers are encouraged to use these less expensive options whenever appropriate to help reduce unnecessary Medicaid spending while maintaining quality of care.
If a prescriber determines that a higher-cost formulation is medically necessary for an individual Medicaid patient, NYRx allows providers to request an exception by submitting a letter of medical necessity, supporting chart notes, and peer-reviewed literature for review. Drugs included in the Cost Optimization Program are identified with an “MR” designation on the NYRx Preferred Drug List when they are also subject to other Medicaid pharmacy programs. The July 16 update adds new medications to the program but does not change the overall policy or exception process.
