Making Care Primary Webinar Now Available
The Making Care Primary webinar conducted on December 6th is now available for viewing at https://youtu.be/h8BjusgeghQ The webinar, sponsored by the Medical Society of the State of New York, the NYS Academy of Family Physicians, and the NY Chapter of the American College of Physicians, provided participants with information about the Making Care Primary payment model and the application process. CMS has extended the application deadline for the Making Care Primary (MCP) Model by two weeks; the new deadline for application submission is Thursday, December 14, at 11:59 p.m. EST.
Nicholas Minter, Director of the Division of Advanced Primary Care, Patient Care Models Group, CMS Innovation Center, and Ms. Kate Davidson, LCSW, Director of Learning and Diffusion Group at Center for Medicare and Medicaid Innovation, provided an overview of the application process during the webinar and answered questions.
Making Care Primary (MCP) Model will be tested in eight states and include practices in Upstate New York (north of Westchester County). Eligible primary care groups include internal medicine, family medicine, and pediatrics. Another unique feature of this model is that FQHCs can also participate.
The MCP application is non-binding and should take no more than 15 minutes to complete. Find eligibility requirements and model details in the RFA. MCP will launch on July 1, 2024, in eight states: Colorado, North Carolina, New Jersey, New Mexico, Upstate New York, Minnesota, Massachusetts, and Washington, and will operate over 10.5 years. This model aims to strengthen coordination between patients’ primary care clinicians, specialists, social service providers, and behavioral health clinicians to drive improved chronic disease prevention, fewer emergency room visits, and better health outcomes.
