As artificial intelligence (AI) continues transforming the healthcare industry, its influence is reshaping the medical residency application process. From personal statement editing to application screening, AI tools are used by students, faculty, and residency program directors alike, raising new questions about equity, ethics, and oversight in one of medicine’s most pivotal transitions.

This year, some residency programs are utilizing AI-assisted platforms, such as Thalamus, which partners with the Electronic Residency Application Service (ERAS), to analyze transcripts, identify applicant-program fit, and streamline high-volume application reviews. With location identified as a significant factor in applicant preferences, AI tools can now flag geographic connections that may signal a candidate’s likelihood of matching and remaining in a given region.

The National Resident Matching Program (NRMP) has clarified that it has not issued guidance on using AI to evaluate applications and reaffirmed its commitment to equitable access for international medical graduates. Meanwhile, the Association of American Medical Colleges (AAMC) has issued formal principles for the responsible use of AI, emphasizing that human judgment should remain central to the selection process.
